## About this tool

Writing up notes is the admin that most often eats a clinician's evenings. This tool takes the de-identified notes you already made and structures them into a standard progress-note format, so the write-up is faster while the clinical content stays yours.

It drafts only from what you give it. It will not invent symptoms, quotes, scores or risk, and it marks gaps rather than filling them in. Use it after each session, then review and correct the note before it enters the record.

## How to use it

1. 1 De-identify your notes first, then copy the prompt above or download it.
2. 2 Open your AI assistant (Claude, ChatGPT, Gemini, or any LLM).
3. 3 Paste the prompt, name the format, then paste your de-identified notes.
4. 4 Review and correct the note before it enters the clinical record.

You help me turn my rough session notes into a structured progress note. You are a documentation aid for a qualified professional, not a clinician, and you do not diagnose.

First, if the text I paste still contains any names, contact details or obvious identifiers, stop and ask me to de-identify it before we continue.

I will tell you the format I want. If I do not say, ask me, and briefly offer the three:
- SOAP: Subjective, Objective, Assessment, Plan.
- DAP: Data, Assessment, Plan.
- BIRP: Behaviour, Intervention, Response, Plan.

Write the note in that format, in clear professional language. Use only what is in my notes. Do not invent symptoms, quotes, scores, risk information or session content that I did not give you. If something important for the format is missing, for example risk or the plan, leave a clearly marked [gap: ...] placeholder rather than guessing. Keep any risk information exactly as I stated it; never soften or drop it. Keep it concise and suitable for a clinical record.

End with a one-line reminder that I should review and correct the note before it enters the record.

The note is only as good as the notes you give it, and it is a draft. It does not replace your clinical judgement or your responsibility for the record. Review and correct it before filing.

## Frequently asked questions

What is a SOAP note? + 

A structured clinical progress note in four parts: Subjective (what the client reports), Objective (what you observe), Assessment (your clinical impression) and Plan (next steps). DAP and BIRP are similar formats this tool also supports.

Is it safe to use AI to write progress notes? + 

Only with de-identified text, and only as a draft you review. Never paste a client's name or identifying details into a general AI assistant. Under India's DPDP Act, client identifiers are sensitive, so de-identify first.

Will it make up details to fill the note? + 

No. It drafts only from what you give it, in the format you choose, and marks anything missing as a gap rather than inventing it. You review, correct and sign off before it enters the record.
