OCD is a private loop: the thought, the fear, the ritual, the relief, and the thought again.

## What is OCD?

Obsessive-compulsive disorder is a loop. An unwanted thought, image or urge arrives, it brings a spike of anxiety or disgust, and you do something to make that feeling go away: wash, check, count, arrange, pray, replay, ask for reassurance. The relief is real and short. Because the ritual worked, the brain learns that the thought was dangerous, and next time it comes back louder.

The thoughts are usually about the things you care about most: harming someone, contamination, doing something immoral, things not being right. Having the thought is not the illness; almost everyone has intrusive thoughts. OCD is what happens when you cannot let one go, and the rituals start taking an hour or more of your day.

In Kerala OCD is still often mistaken for being religious, careful or perfectionist, and people wait years before anyone names it. It is one of the most treatable conditions we see, and the treatment is specific: not general talk therapy, but ERP.

### Common signs of OCD

Unwanted thoughts or images that keep returning and feel shameful or frightening Washing, cleaning or checking far beyond what the situation needs Needing things symmetrical, ordered, or "just right" before you can move on Mental rituals: counting, praying, repeating phrases, reviewing past events Asking family for reassurance again and again, or avoiding places and objects Knowing the fear is excessive but feeling unable to stop

## Types of OCD we treat

OCD wears many costumes. The treatment underneath is the same.

[**Contamination OCD** Fear of germs, illness or dirt, with washing and avoidance.](/tools/ocd/) [**Checking OCD** Doors, stoves, taps, messages, checked and re-checked for a harm that never comes.](/tools/ocd/) [**Harm and intrusive-thought OCD** Violent, sexual or blasphemous thoughts that horrify the person having them.](/tools/ocd/) [**Symmetry and "just right" OCD** Arranging, ordering, repeating until an inner sense of completeness arrives.](/tools/ocd/) [**Religious and moral OCD (scrupulosity)** Fear of having sinned or offended, with prayer, confession or mental review as the ritual.](/tools/ocd/) [**Relationship OCD** Constant doubt about whether you love your partner or they love you, with checking and reassurance-seeking.](/tools/relationship-health/)

## How OCD treatment works

The first-line psychological treatment for OCD is exposure and response prevention (ERP), a structured form of CBT. With your psychologist you build a ladder of feared situations, face them one step at a time, and, this is the part that matters, do not perform the ritual. The anxiety rises, peaks, and then falls on its own. Repeated enough, the brain learns the feared outcome does not happen and the thought loses its power. Most people see meaningful change in 12 to 20 sessions.

Map the loop: your specific obsessions, rituals, avoidance and the reassurance you seek Build the exposure ladder together, from easiest to hardest, at a pace you agree to Practise ERP in sessions and between them, with your psychologist coaching you through the urge Treat what rides along, usually anxiety or low mood, and plan for relapse prevention

Sessions are 50 minutes, over encrypted video, in Malayalam or English. New to therapy? Here is [what to expect in your first session](/what-to-expect-first-session/), and why the [bond with your therapist](/therapeutic-alliance/) matters more than any technique.

## What our clients say

Starting couple therapy with Teresa has been one of the best decisions for us. Therapy helped me calm down, reflect on my own patterns, and recognize how both of us were struggling and growing together. It truly brought more clarity, understanding, and peace into our relationship.

I had tried several therapy sessions in the past without seeing any noticeable changes in my thought patterns. Then, I decided to give Ello Mind a try. My therapist, Teresa, is very friendly, and I was able to connect with her perspective right away. She has already helped me make a positive change during one of the lowest points in my life.

I’ve been attending therapy sessions with Hyma ma’am, and the experience has been incredibly positive. She creates a safe and comfortable environment, allowing me to open up and explore my thoughts at my own pace. Her approach is both professional and compassionate. I’m truly grateful for the progress I’ve made with her guidance.

I had a great experience with Ellomind. I worked with Teresa and she is a wonderful therapist. Helped me get my thoughts sorted and gave some effective exercises that helped me on my mental wellness journey. I am giving this feedback a few months after my sessions ended and still feel great. The effects of the therapy are long lasting.

Teresa was able to correctly identify that I had issues with confidence and assertiveness using the right questions and exercises. She then taught me CBT frameworks and techniques that helped me improve my self-image. Teresa is very open to receiving feedback and integrating it into subsequent sessions.

I’ve been taking sessions at Ello Mind since 2023, and Teresa has helped me overcome many things. She doesn’t just give advice — she helps you understand yourself better. She listens patiently and really tries to understand what you’re going through. I genuinely appreciate her work and highly recommend Ello Mind.

## Frequently asked questions

What is the best treatment for OCD? +

Exposure and response prevention (ERP), a specific form of cognitive behavioural therapy, has the strongest evidence and is the first-line treatment in every major guideline. SSRIs help many people and combine well with ERP. Generic talk therapy, relaxation or simply trying to reason the thoughts away do not work for OCD and can make the loop stronger.

What is ERP therapy and does it hurt? +

ERP means facing the thing that triggers the obsession (exposure) and not doing the ritual (response prevention). It is uncomfortable by design, but it is gradual and always agreed with you in advance. Nobody is thrown in at the deep end. The discomfort peaks and then falls, and each time it falls a little sooner. That falling is the treatment working.

Can OCD be cured? +

OCD can be treated to the point where it no longer runs your life, and many people reach that. Intrusive thoughts may still show up occasionally, because they show up for everyone, but they stop carrying weight and the rituals stop. Relapse-prevention skills are part of the treatment so you know what to do if it flares.

Can OCD be treated without medication? +

Yes, for many people. ERP alone is effective for mild to moderate OCD. For severe OCD, or when ERP alone is not enough, medication prescribed by a psychiatrist can make the therapy easier to do. Your psychologist will tell you honestly which situation you are in.

Does online ERP work? +

Yes. Studies of internet-delivered and video-delivered ERP show outcomes comparable to in-person therapy, and online sessions have a practical advantage: you can do exposures in the actual place the OCD lives, your own kitchen, bathroom or bedroom, with your psychologist on the call.

Is it OCD or am I just careful? +

Liking things clean or double-checking a door is normal. It leans towards OCD when the thoughts are unwanted and distressing, the rituals take an hour or more a day, you feel unable to stop, and it is costing you time, relationships or peace. The free [OCI-R OCD test](/tools/ocd) gives you a validated score in three minutes; 21 or above is the usual point to talk to someone.

My family keeps reassuring me. Is that helping? +

It feels like help, but reassurance is a ritual done by someone else, and it keeps the loop going. Part of OCD treatment is often a session with family so they understand how to support you without feeding the compulsion. Many Malayali families find this the most useful hour of the whole process.

Can I have my OCD sessions in Malayalam? +

Yes. All our psychologists speak Malayalam and English, and describing intrusive thoughts in your own language often makes them easier to say out loud. Sessions are online from Kerala, the rest of India, or the Gulf. See [online counselling in Malayalam](/malayalam-counselling-online).

How much does OCD therapy cost? +

An individual session is ₹1,499, or from ₹1,049 per session with a multi-session plan. Each session is 50 minutes. You pay after your session. [View all plans](/pricing).
