How depression often presents in Kerala
Two patterns are worth naming, because they delay treatment.
It arrives as a physical complaint. Body aches, fatigue, headaches, stomach trouble. Many people see three or four doctors and have every test come back normal before anyone asks about mood. This is common and it is not the patient's fault.
It gets read as a character problem. Someone who cannot get out of bed is described as മടിയൻ, lazy. Someone who has withdrawn is described as arrogant. The illness gets moralised, the person gets ashamed, and the shame delays help further.
The distance between "he has become lazy" and "he has been ill for eight months" is often the distance between a family and treatment.
