What is OCD?
Obsessive-compulsive disorder is a loop. An unwanted thought, image or urge arrives, it brings a spike of anxiety or disgust, and you do something to make that feeling go away: wash, check, count, arrange, pray, replay, ask for reassurance. The relief is real and short. Because the ritual worked, the brain learns that the thought was dangerous, and next time it comes back louder.
The thoughts are usually about the things you care about most: harming someone, contamination, doing something immoral, things not being right. Having the thought is not the illness; almost everyone has intrusive thoughts. OCD is what happens when you cannot let one go, and the rituals start taking an hour or more of your day.
In Kerala OCD is still often mistaken for being religious, careful or perfectionist, and people wait years before anyone names it. It is one of the most treatable conditions we see, and the treatment is specific: not general talk therapy, but ERP.

How OCD treatment works
The first-line psychological treatment for OCD is exposure and response prevention (ERP), a structured form of CBT. With your psychologist you build a ladder of feared situations, face them one step at a time, and, this is the part that matters, do not perform the ritual. The anxiety rises, peaks, and then falls on its own. Repeated enough, the brain learns the feared outcome does not happen and the thought loses its power. Most people see meaningful change in 12 to 20 sessions.

Exposure and response prevention (ERP), a specific form of cognitive behavioural therapy, has the strongest evidence and is the first-line treatment in every major guideline. SSRIs help many people and combine well with ERP. Generic talk therapy, relaxation or simply trying to reason the thoughts away do not work for OCD and can make the loop stronger.
ERP means facing the thing that triggers the obsession (exposure) and not doing the ritual (response prevention). It is uncomfortable by design, but it is gradual and always agreed with you in advance. Nobody is thrown in at the deep end. The discomfort peaks and then falls, and each time it falls a little sooner. That falling is the treatment working.
OCD can be treated to the point where it no longer runs your life, and many people reach that. Intrusive thoughts may still show up occasionally, because they show up for everyone, but they stop carrying weight and the rituals stop. Relapse-prevention skills are part of the treatment so you know what to do if it flares.
Yes, for many people. ERP alone is effective for mild to moderate OCD. For severe OCD, or when ERP alone is not enough, medication prescribed by a psychiatrist can make the therapy easier to do. Your psychologist will tell you honestly which situation you are in.
Yes. Studies of internet-delivered and video-delivered ERP show outcomes comparable to in-person therapy, and online sessions have a practical advantage: you can do exposures in the actual place the OCD lives, your own kitchen, bathroom or bedroom, with your psychologist on the call.
Liking things clean or double-checking a door is normal. It leans towards OCD when the thoughts are unwanted and distressing, the rituals take an hour or more a day, you feel unable to stop, and it is costing you time, relationships or peace. The free OCI-R OCD test gives you a validated score in three minutes; 21 or above is the usual point to talk to someone.
It feels like help, but reassurance is a ritual done by someone else, and it keeps the loop going. Part of OCD treatment is often a session with family so they understand how to support you without feeding the compulsion. Many Malayali families find this the most useful hour of the whole process.
